Kona Medical Consulting and The Home Health Team is actively looking for a Home Health Field Registered Nurse (RN) to service various Texas areas, delivering skilled nursing in patients' homes.

The ideal candidate will perform in-home assessments, develop care plans, manage wounds and medications, coordinate with physicians and case managers, and maintain accurate documentation while providing compassionate, patient‑centered care.

Qualificações

• Current RN license in the state of Texas.
• 1+ year of nursing experience (Home Health preferred but not required).
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation, valid driver’s license, and proof of auto insurance.
• Comfortable using EMR/mobile charting.
• Compassionate, professional, and patient-focused.

Responsabilidades

• Perform in-home patient assessments and skilled nursing visits
• Develop and update individualized care plans
• Perform wound care, disease management, medication administration, and patient education
• Communicate effectively with physicians, case managers, and interdisciplinary teams
• Complete accurate, timely documentation in accordance with regulatory requirements
• Participate in on-call rotation as needed
• Build trusting relationships with patients and families
