AccentCare in Tacoma, Washington is seeking a Clerical Territory Patient Admissions Coordinator to establish and maintain communication with clients, families, referral sources, and agency staff within the assigned territory. This role coordinates referrals, ensures documentation, and supports timely acceptance based on agency criteria.

Requirements include a high school diploma or GED, at least one year in home care or hospice, and prior EMR use with HCHB preferred.

Qualifications

• High School or GED.
• Minimum of one year experience in home care, hospice, or personal care services, required.
• Prior use of EMR, HCHB preferred.
• Experience with Medicare/Medicaid or commercial payors preferred.

Responsabilités

• Collaborates with Referral Source, Sales, Clients and agency operations to collect information for a quality referral.
• Documents referral information, actions, and follow-up in the medical record within established timeframes.
• Manages referrals from platforms such as eFax, phone, or referral portal/Hospital EMR portal.
• Reviews Medicare client eligibility alerts and responses where applicable.
• Requests, H&P, Discharge Summary and other documents needed for ICD-10 coding by the CPOCC team.
