Job summary

AccentCare, Inc. is seeking a Clerical Territory Patient Admissions Coordinator in Burnsville, MN. This full-time, on-site role supports referral intake, documentation, and collaboration with care teams to ensure timely admissions and high-quality patient referrals.

The ideal candidate has a high school diploma, at least one year of home care experience, and familiarity with EMR systems (HCHB preferred). Knowledge of Medicare/Medicaid payors is a plus.

Qualifications

• High School or GED is required.
• At least one year of home care, hospice, or personal care experience.
• Experience with EMR systems; HCHB preferred.
• Experience with Medicare/Medicaid payors recommended.

Responsibilities

• Collaborates with Referral Source, Sales teams, Clients and local agency operations to collect information for a quality referral.
• Documents referral information and actions in the medical record within established time frames.
• Ensures timely attention to referrals from eFax, phone, or referral portal/Hospital EMR portal.
• Reviews Medicare client Eligibility Alerts and Responses when applicable.
• Requests H&P, Discharge Summary, and other documents for ICD-10 coding by the CPOCC team.
• Completes follow-up calls to confirm information and clarify out-of-pocket expenses.
