AccentCare in Grants Pass, Oregon is seeking a Clerical Territory Patient Admissions Coordinator to establish and maintain communication with clients, families and referral sources within a defined territory. This role partners with agency staff, referral sources, and sales to ensure timely acceptance of referrals and complete documentation for quality referrals.

This full-time, Monday–Friday 8am–5pm position requires a high school diploma, minimum one year in home health, hospice or personal

Qualificações

• High School diploma or GED required.
• Minimum one year experience in home care, hospice, or personal care services.
• Prior use of EMR systems; HCHB preferred.
• Experience dealing with Medicare, Medicaid, and commercial payors preferred.

Responsabilidades

• Collaborates and maintains effective communication with the Referral Source, Sales teams, Clients and/or local agency operations to collect needed information to process a quality referral.
• Documents all referral related information, actions taken, and follow-up required in the medical record in a timely and accurate manner. As applicable utilizes the agency's software solution to facilitate the referral process by completing required workflow task within the Company's established time frames.
• Ensures timely attention to referrals received from a variety of platforms, e.g., eFax, telephone, or referral portal/Hospital EMR portal.
• Where applicable, reviews Eligibility Alerts and Responses for Medicare Clients.
• Requests, H&P, Discharge Summary, and other documents needed for ICD 10 coding by the CPOCC team, if not received with the referral information.
• Completes a "Follow Up" call to the Client/Caregiver/Referral Source to collect or confirm information. Example: confirms out of pocket expense, clarify teachable caregiver, and collect missing demographic and physician information.
