Qualifikationen

• High School or GED
• Minimum of one year experience in home care, hospice, or personal care services
• Prior use of Electronic Medical Records (EMR), HCHB preferred
• Experience dealing with Medicare/Medicaid and commercial payers preferred

Aufgaben

• Collaborates and maintains communication with Referral Source, Sales, Clients and local operations to collect needed information to process a quality referral.
• Documents referral information, actions taken, and follow-up in the medical record within required timeframes.
• Ensures timely attention to referrals from eFax, telephone, or portal/Hospital EMR.
• Reviews Eligibility Alerts and Responses for Medicare Clients where applicable.
• Requests H&P, Discharge Summary and other documents needed for ICD 10 coding by the CPOCC team.
• Performs follow-up calls to collect or confirm information including out-of-pocket expenses.
