Job summary

AccentCare Inc. is seeking a Territory Patient Admissions Coordinator to establish and maintain communications with clients, families, and referral sources within an assigned territory.

This role partners with agency staff, referral sources, and sales to ensure timely acceptance of referrals and quality documentation. Medical backoffice experience is required, and the position is located onsite in Chisago City, MN.

Qualifications

• High School or GED required.
• Minimum of one year experience in home care, hospice, or personal care services.
• Experience with Electronic Medical Records (EMR), HCHB preferred.
• Experience dealing with Medicare/Medicaid and other payors preferred.

Responsibilities

• Collaborates with Referral Source, Sales, Clients and local operations to collect information for a quality referral.
• Documents referral information, actions taken, and follow-up in the medical record.
• Ensures timely attention to referrals via eFax, phone, or portal.
• Reviews Medicare Eligibility Alerts and Responses where applicable.
• Requests and obtains H&P, Discharge Summary, and other documents for ICD-10 coding.
• Performs follow-up calls to collect or confirm information and clarify details.
