Job summary

AccentCare, Inc. in Tacoma, WA is seeking a Clerical Territory Patient Admissions Coordinator to manage referrals and work with clients, families and referral sources in the assigned territory.

The role collaborates with agency staff, referral sources and sales to ensure timely referral acceptance, gather required information and document in the EMR for quality referrals. This is a full-time position with a Monday–Friday schedule and a salary of $27.00–$30.00 per hour.

Qualifications

• High school diploma or GED required.
• Minimum of one year experience in home care, hospice, or personal care services.
• Prior use of EMR systems (HCHB preferred).
• Experience dealing with Medicare, Medicaid, and commercial payers is a plus.

Responsibilities

• Collaborates with referral sources, sales teams, clients and local agency operations to collect information for a quality referral.
• Documents referral-related information, actions taken and follow-ups in the medical record within stated time frames.
• Ensures referrals are attended to promptly from eFax, phone, or portal sources.
• Reviews eligibility alerts and responses for Medicare clients where applicable.
• Requests necessary documents (H&P, discharge summaries) for ICD-10 coding by the CPOCC team.
• Makes follow-up calls to confirm information and clarify patient or caregiver details.

What You Need to Know

The Territory Patient Admission Coordinator establishes and maintains communication with the clients, family members and referral sources, in a given territory. This role is responsible for partnering with agency staff, referral sources, and sales to achieve optimal patient satisfaction & outcomes. The Territory Patient Admission Coordinator is responsible for the timely acceptance or decline of the referral based off the predetermined criteria set by the Agency and facilitates obtaining all information and documentation necessary to complete a quality referral. This position’s assigned territory, may encompass one or more states in AccentCare’s operation and may change with the needs of the business.

• Prior use of Electronic Medical Records (EMR), HCHB preferred
• Experience in dealing with a variety of payors in healthcare, e.g., Medicare, Medicaid, and commercial payers, preferred

Responsiblities

• Collaborates and maintains effective communication with the Referral Source, Sales teams, Clients and/or local agency operations to collect needed information to process a quality referral.
• Documents all referral related Information, actions taken, and follow-up required in the medical record in a timely and accurate manner. As applicable utilizes the agency’s software solution to facilitate the referral process by completing required workflow task within the Company’s established time frames.
• Ensures timely attention to referrals received from a variety of platforms, e.g., eFax, telephone, or referral portal/Hospital EMR portal.
• Where applicable, reviews Eligibility Alerts and Responses for Medicare Clients.
• Requests, H&P, Discharge Summary, and other documents needed for ICD 10 coding by the CPOCC team, if not received with the referral information.
• Completes a “Follow Up” call to the Client/Caregiver/Referral Source to collect or confirm information. Example: confirms out of pocket expense, clarify teachable caregiver, and collect missing demographic and physician information.
