Job summary

AccentCare in San Diego, CA is seeking a Territory Patient Admissions Coordinator to manage referrals and coordinate with clients, families, and referral sources within a defined territory. This full-time role partners with agency staff, sales, and clinical teams to ensure timely, accurate patient admissions and quality referrals.

You will handle documentation, review eligibility, and collect required information to support ICD-10 coding and patient care transitions. On-site work required.

Qualifications

• Minimum of one year experience in home care, hospice, or personal care services.
• Experience dealing with Medicare, Medicaid, and commercial payors preferred.
• EMR experience with HCHB preferred.

Responsibilities

• Collaborates with Referral Source, Sales teams, Clients and local operations to collect info for a quality referral.
• Documents referral information, actions, and follow-up in the medical record; uses agency software for workflow within set time frames.
• Ensures timely attention to referrals from eFax, phone, or portal/Hospital EMR.
• Reviews Eligibility Alerts and Responses for Medicare clients where applicable.
• Requests H&P, Discharge Summary and other documents for ICD-10 coding by CPOCC team.
• Completes Follow Up calls to client/caregiver/referral source to confirm details.
