Responsibilities

• Collaborate and maintain communication with referral sources and local agency.
• Document referral information in medical records accurately.
• Ensure timely attention to received referrals.
• Compile necessary documents for ICD-10 coding.
• Conduct follow-up calls to confirm information with clients.

Skills
