Qualifications

• High School or GED required.
• Minimum of one year experience in home care, hospice, or personal care services.
• Prior use of Electronic Medical Records (EMR), HCHB preferred.
• Experience dealing with Medicare/Medicaid and commercial payers is preferred.

Responsibilities

• Collaborates with Referral Source, Sales, Clients to collect needed information for quality referrals.
• Documents referral actions and follow-ups in the medical record within established timeframes.
• Attends referrals from eFax, phone, or portal; ensures timely processing.
• Requests and coordinates necessary documents (H&P, Discharge Summary, ICD coding supports).
