Job summary

AccentCare is seeking a Clerical Territory Patient Admissions Coordinator to establish and maintain communication with clients, family members, and referral sources within a designated territory. The role supports agency staff, referral sources, and sales to ensure timely, quality referrals and optimal patient outcomes.

The position requires medical backoffice experience and will involve coordinating with payors and healthcare teams. Candidates should be prepared to work onsite in St.

Qualifications

• High School or GED required.
• Minimum of one year experience in home care, hospice, or personal care services.
• Prior use of Electronic Medical Records (EMR); HCHB preferred.
• Experience with Medicare/Medicaid and commercial payors preferred.

Responsibilities

• Collaborates with Referral Source, Sales teams, Clients and/or local agency operations to collect necessary information to process a quality referral.
• Documents referral information, actions taken, and follow-up in the medical record in a timely and accurate manner.
• Uses the agency software to complete required workflow tasks within established timeframes.
• Ensures timely attention to referrals from eFax, telephone, or referral portal/Hospital EMR portal.
• Reviews Eligibility Alerts and Responses for Medicare Clients when applicable.
• Requests H&P, Discharge Summary, and other documents needed for ICD 10 coding by the CPOCC team.
• Executes follow-up calls to clients/caregivers/referral sources to confirm information and clarify details.
