Overview

Territory Patient Admissions Coordinator

Pay: $28-$32 per hour, based on experience

Schedule: Full-time | Monday – Friday | 8 a.m. to 5 p.m. | One weekend per month

The Territory Patient Admissions Coordinator serves a defined territory and builds strong relationships with clients, caregivers, referral sources, and internal teams to support timely, high-quality admissions. By carefully reviewing referrals against established criteria and gathering complete documentation, the Territory Patient Admissions Coordinator ensures each patient is set up for the best possible care journey. Working closely with agency staff and sales partners, this position helps drive positive outcomes and exceptional satisfaction. With a territory that may span multiple states and evolve, this role offers meaningful impact, collaboration, and the opportunity to support patients at an important moment in their care experience.

What You Need to Know
Territory Patient Admissions Coordinator Qualifications
  • Home health experience required
  • Medical back office experience required

  • Experience in dealing with a variety of payors in healthcare, including Medicare, Medicaid, and commercial payers preferred
Territory Patient Admissions Coordinator Responsibilities
  • Collaborates and maintains effective communication with the Referral Source, Sales teams, Clients, and/or local agency operations to collect needed information to process a quality referral.
  • Documents all referral-related information, actions taken, and follow-up required in the medical record in a timely and accurate manner. As applicable, utilizes the agency’s software solution to facilitate the referral process by completing required workflow tasks within the Company’s established timeframes.
  • Ensures timely attention to referrals received from a variety of platforms, e.g., eFax, telephone, or referral portal/Hospital EMR portal.
  • Reviews Eligibility Alerts and Responses for Medicare Clients.
  • Requests H&P, Discharge Summary, and other documents needed for ICD-10 coding by the CPOCC team, if not received with the referral information.
  • Completes a "Follow Up" call to the Client/Caregiver/Referral Source to collect or confirm information. Example: confirms out-of-pocket expenses, clarifies the teachable caregiver, and collects missing demographic and physician information.
