AccentCare, Inc. in Oakdale, MN is seeking a Clerical Territory Patient Admissions Coordinator to manage referrals and coordinate with agencies to ensure timely admissions.

The role is full-time, on-site only, with hourly pay of $23.00 to $30.00 and requires medical backoffice EMR experience; familiarity with Medicare/Medicaid payors is a plus. You will collaborate with referral sources, sales, and care teams, document in the EMR, and help maintain patient satisfaction across the assigned

Qualifications

• High School or GED completed.
• One year plus experience in home care, hospice, or personal care services.
• Experience with EMR systems and payor handling preferred.

Responsibilities

• Collaborates and maintains effective communication with Referral Source, Sales teams, Clients and local operations to collect information for a quality referral.
• Documents referral information, actions, and follow-up in the medical record within established timeframes.
• Ensures timely attention to referrals from eFax, phone, or portal/Hospital EMR.
• Requests H&P, Discharge Summary, and other documents for ICD-10 coding when not received with referral.
