Job summary

AccentCare Inc. is seeking a Territory Patient Admissions Coordinator Clinical for the Burnsville, MN area.

This on-site role manages the timely acceptance or decline of referrals within an assigned territory, coordinating with clients, families and referral sources to ensure admissions align with criteria and patient needs. The position requires a licensed RN, PT, OT, LPN, PTA or COTA and 3–7 years of experience in healthcare relations, with EMR familiarity to support streamlined patient intake

Qualifications

• Must be licensed RN, PT, OT, LPN, PTA or COTA.
• 3–7 years of experience in facility/physician relationships; discharge processes preferred.
• Minimum of one year experience in home care, hospice, or personal care services preferred.
• Experience with Medicare, Medicaid, and commercial payors preferred.

Responsibilities

• Collaborates and maintains effective communication with Referral Source, Sales teams, Clients and local agency operations to collect needed information to process a quality referral.
• Obtains and documents verbal orders for patient evaluation as required in the state where the Coordinator is licensed.
• Documents referral information, actions taken, and follow-up required in the medical record using the agency’s software solution.
• Ensures timely attention to referrals received from eFax, telephone, or referral portal/Hospital EMR portal.
• Reviews Eligibility Alerts and Responses for Medicare Clients where applicable.
• Requests H&P, Discharge Summary, and other documents needed for ICD 10 coding by the CPOCC team, if not received with the referral information.
