Qualifications include active RN or LVN license, 3–7 years in facility/physician relationships, plus 1 year in home care preferred. EMR experience and knowledge of Medicare/Medicaid payors are valued. The position is full-time and onsite.

Qualifications

• Licensed LVN or RN in practicing state.
• Minimum of 1 year experience in home care, hospice, or personal care services.
• Experience with Electronic Medical Records (EMR) preferred.
• Experience dealing with Medicare/Medicaid and commercial payers preferred.
• 3–7 years of facility/physician relationship experience preferred.

Responsibilities

• Collaborates with Referral Source, Sales, Clients and local operations to collect information to process a referral.
• Obtains and documents verbal orders for patient evaluation as required by state license.
• Documents all referral information and actions; uses agency software to process referrals within set timeframes.
• Ensures timely attention to referrals from channels like eFax, phone, or portal/Hospital EMR.
