AccentCare, Inc. is seeking a Territory Patient Admissions Coordinator Clinical to manage referrals and admissions in Grants Pass or Medford, OR.

This on-site, full-time role supports timely acceptance or decline of referrals across multiple facilities and payors. The ideal candidate is a licensed RN or LVN with home health exposure, strong communication skills, EMR experience, and the ability to collaborate with referral sources, agency staff, and sales teams to ensure optimal patient

Qualifications

• Licensed RN or LVN in practicing state.
• 3-7 years of experience in facility/physician relationships.
• Minimum of one year experience in home care, hospice, or personal care services.

Responsabilités

• Collaborates and maintains effective communication with Referral Source, Sales teams, Clients and/or local agency operations to collect needed information to process a quality referral.
• Obtains and documents verbal orders for patient evaluation as required by state.
• Documents all referral related information, actions taken, and follow-up in the medical record and uses the agency’s software to complete workflow tasks in established time frames.
• Ensures timely attention to referrals from eFax, telephone, or portal/EMR referrals.
• Reviews Medicare/Medicaid eligibility alerts and responses where applicable.
• Requests H&P, Discharge Summary, and other ICD-10 documents for the CPOCC team if not included with the referral.
