AccentCare is seeking a Territory Patient Admissions Coordinator Clinical to manage referrals across the Grants Pass/Medford area. You will coordinate with referral sources, sales teams and agency staff to ensure timely admission decisions and complete documentation.

The role requires a valid nursing license (LVN/RN) with 1+ year in home care or hospice, EMR experience, and strong communication across providers. Office hours are Monday–Friday, with onsite work in Grants Pass or Medford, Oregon.

Qualificações

• Licensed LVN or RN in practicing state.
• 3–7 years of experience in facility/physician relationships.
• Minimum one year in home care, hospice, or personal care services.

Responsabilidades

• Collaborates with Referral Source, Sales teams, Clients to collect information for a quality referral.
• Documents verbal orders for patient evaluation per state requirements.
• Maintains medical records timely and accurately in the agency system.
