Job summary

Talent With Heart in NYC, Long Island, and Westchester seeks a Business Development Specialist to expand referral networks and increase patient access to home care services.

You will build relationships with physicians, hospitals, and community organizations, educate partners on home care options, and work with the intake team to convert referrals into enrollments. Full-time, on-site with growth potential.

Qualifications

• Experience in home care marketing, community healthcare outreach, long-term care, or healthcare business development.
• Proven ability to generate referrals, enroll patients, or starts of care.
• Strong communication, presentation, and relationship-building skills.
• Willingness to travel locally and manage an assigned territory.

Responsibilities

• Build and maintain relationships with physicians, hospitals, rehabilitation centers, community organizations, and other referral sources.
• Identify and develop new referral opportunities throughout the assigned territory.
• Represent the agency at community events, healthcare offices, and outreach initiatives.
• Educate referral partners on available home care services.
• Collaborate with the intake team to ensure a smooth transition from referral to patient enrollment.
• Drive consistent referral activity that converts into new patient starts of care, meeting monthly growth goals.
• Manage your territory independently while maintaining regular outreach and follow-up activities.
