The manager coordinates care, coaches staff, and ensures compliance with state and federal regulations, while serving as a key liaison across interdisciplinary teams and leadership to maintain high care standards.

Qualifikationen

• Must graduate from an approved school of professional nursing and be licensed to practice as a registered nurse in the operating state.
• 3–5 years of RN experience.
• Experience supervising RN services in home, community, or clinic settings preferred.
• CPR certification required.
• Must have a valid driver’s license and reliable vehicle, with ability to travel to locations.

Aufgaben

• Oversee patient care operations and supervise clinical staff.
• Lead CHAP and regulatory survey readiness and drive QAPI initiatives.
• Monitor patient satisfaction and manage complaint resolutions.
• Coordinate care and develop individualized care plans with an interdisciplinary team.
• Ensure care is delivered safely, effectively, and in compliance with regulations.
• Provide leadership and backup support for PCM and CSS roles as needed.
