Job summary

RON SUNSHINE ASSOCIATES LLC seeks an experienced Clinical Manager to oversee all patient care services and personnel in a California home health setting. You will coordinate patient care, supervise clinical staff, and ensure safety and effectiveness of care while driving CHAP compliance and survey readiness.

You will lead QAPI initiatives, monitor patient satisfaction and complaints, and collaborate with interdisciplinary teams to optimize care plans and transitions.

Qualifications

• Graduate from an approved school of professional nursing and licensed to practice as a registered nurse.
• 5 yrs RN experience with 2 yrs+ supervisor experience in Home Health.
• Must possess and maintain valid CPR certification.
• Must be a licensed driver with insured vehicle and able to travel to all locations.

Responsibilities

• Oversee day-to-day clinical and administrative home health operations.
• Ensure care delivery aligns with professional standards and regulations.
• Provide leadership and backup support as acting leader in absence of Territory Administrator.
• Coordinate patient care with interdisciplinary team and develop care plans.
• Drive quality assessment, track patient needs, and manage transfers/discharges.
• Manage OASIS and discharge data, including editing/locking forms.
• Lead QAPI initiatives and track incidents with corrective actions.
• Maintain accreditation and survey readiness and manage HHCAHPS distribution.
• Lead Emergency Preparedness Planning and post-activation activities.
