Aspire Talent Solutions is seeking a Clinical Manager to lead patient care services, supervise clinical staff, and ensure services meet safety and regulatory standards. The role focuses on CHAP readiness, QAPI initiatives, and ongoing patient satisfaction efforts across multiple locations.

The position requires RN licensure in the operating state, 5+ years of RN experience with at least 2 years in Home Health supervision, CPR certification, and a valid driver's license with travel willingness.

Qualifikationen

• Registered nurse licensed to practice in the state of operation.
• 5+ years RN experience with 2+ years supervision in Home Health.
• Experience managing RN services in home, community, or clinic.
• Must maintain CPR certification.
• Must have a valid driver’s license and insured vehicle; travel to locations as needed.

Aufgaben

• Oversee day-to-day clinical and administrative office operations.
• Coordinate patient care with interdisciplinary team to develop care plans.
• Direct care to align with standards, agency policies, and regulations.
• Lead QAPI initiatives and track incidents and corrective actions.
• Maintain survey readiness and oversee HHCAHPS survey processes within SHP.
• Oversee Emergency Preparedness Planning (EPP) activities and monitoring.
• Provide leadership and backup coverage for PCM and CSS roles as needed.
• Review discharge data and edit/lock Discharge OASIS forms.
