Job summary

CVS Health is seeking a Case Management Coordinator for a full-time, telework position with travel in Northwest Illinois counties. The role focuses on coordinating care plans, educating members, and ensuring access to benefits and services.

The ideal candidate has 2+ years in Behavioral Health or related fields, travel readiness, and experience with electronic documentation and MS Office. A Bachelor's or non-licensed Master's in Behavioral Health or Human Services is required.

Qualifications

• 2+ years experience in Behavioral Health, Social Services or appropriate related field.
• Must be willing and able to travel 50-75% of the time in Winnebago, Boone, Stephenson, Ogle and Jo Daviess Counties, Illinois. Reliable transportation required.
• Must have 2+ years of experience of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluate members' needs and eligibility to determine care resolution and plan.
• Identify high risk factors and coordinate appropriate referrals to case management or crisis intervention.
• Coordinate and implement assigned care plan activities and monitor progress.
• Collaborate with Medical Directors and other health programs to overcome barriers and achieve optimal outcomes.
• Identify and escalate quality of care issues through established channels.
• Use negotiation to secure appropriate options and services to meet member needs.
• Engage members to promote lifestyle/behavior changes and maximize engagement.
• Provide coaching and support for informed medical and healthy lifestyle choices.
• Participate in ongoing team meetings and peer mentoring.
• Support member participation with their provider in healthcare decision-making.
• Document and monitor care per regulatory guidelines.
