Responsibilities
- Coordinate and monitor care plans and services for members.
- Schedule appointments and assist with access to benefits.
- Educate members about programs and resources available.
- Collaborate with case managers, providers and teams.
- Identify high-risk cases and refer for higher level care when needed.
- Document care activities in accordance with guidelines.
