Job summary

CVS Health Corporation is seeking a remote Clinical Case Manager to support members across the United States, working EST hours from home. The role partners with a registered nurse case manager to assess, plan, and advocate for member benefit plans and health needs.

Responsibilities include researching options and resources and empowering members to make positive lifestyle and medical choices, all while collaborating with clinical staff to connect members with community resources.

Qualifications

• Must live in North Carolina.
• Master's Degree in Social Work.
• Active, unrestricted licensure as one of the following: LSW, LMSW, LCSW, or LISW.
• 2+ years of experience in medical social work or case management.
• Ability to obtain multi-state licensure (min 10 states) before new hire training.

Responsibilities

• Research options and resources for members with financial, transportation, safety and housing needs.
• Empower members to make positive lifestyle and medical choices.
• Collaborate with nurses and other clinical staff to assist members with community resources.
• All member contact is done telephonically or via electronic exchange.
