MindCare Solutions Group, Inc. is seeking a Telehealth Care Navigation Coordinator to act as a liaison between care teams and partner sites. The role schedules, coordinates clinical activities, and manages information exchange across sites and physicians via telemedicine.

The position is PRN with no regular days, covering across shifts, including holidays. Compensation is $25/hour, with flexibility required to meet staffing needs and holiday coverage across multiple facilities.

Qualifications

• Two+ years of practice administration experience.
• Experience with telehealth technology.
• Knowledge of medical service lines preferred.

Responsibilities

• Facilitate safe and effective clinical consultations.
• Coordinate reporting of medical records, schedules, billing, utilization, and outcomes.
• Manage calendars and appointments, including chart prep and documentation delivery to customers.
• Operate telemedicine platforms, video conferencing, and EMR systems.
• Support scheduling to optimize clinical time and communicate provider changes.
