Job summary

VieMed Healthcare seeks a dedicated Care Coordinator to assess patients’ needs via phone or teleportal, collaborate with a clinical team, and connect patients with community resources while maintaining thorough EHR documentation.

The role emphasizes telehealth experience, strong listening skills, and navigating the medical insurance process; some travel is required, with hours 8:00 a.m.–5:00 p.m., Monday–Friday.

Qualifications

• Experience with medical insurance and community resources.

Responsibilities

• Determines patients’ current needs to enhance daily living via telephonic call or teleportal.
• Based on identified needs, assists the patient with acquiring necessary community resources.
• Maintains an EHR of the patient’s progress by documenting patient/agent interaction and progress or lack thereof.
• Develops a professional relationship with patients and caregivers to foster positive patient outcomes.
• Works effectively with Clinical Team.
• Coordinates and executes activities resulting in keeping Viemed patients on service with respiratory equipment.
• Maintains operations by following policies and procedures, participating in quality reviews and reporting needed changes.
• Complies with federal, state, and local legal requirements advising management on needed actions.
• Maintains client confidence and protects operations by keeping information confidential.
• Participates in reauthorization calls and assists patients with identified needs.
• Attends mandatory staff meetings.
• Attends in-services to enhance knowledge base for providing services.
• Assists patients with navigating medical options within the medical system.
• Maintains an understanding of the medical insurance process.
• Monitors 02 escalations, prevoid, and pick up reports to identify needs.
• Maintains proficiency in Therapy Notes, Bonafide, Smartsheet, Excel, One Source, Health Gorilla, and Tableau.
• Other responsibilities and projects assigned.
• Must demonstrate high ethical standards and integrity in all aspects of the role.
• Some travel is required.
