Pear Suite is a digital health company providing care navigation software to support community health workers, doulas, care navigators, and their organizations across the U.S. We seek a Nurse Practitioner to conduct telehealth assessments, enable access to CHW services, and coordinate care with a person-centered, whole-person approach.

This PRN role offers flexible hours with a clear path toward more regular part-time or full-time work as patient volume grows, with an initial hourly rate of $80.

Qualifications

• Active NP license in state(s) where practice will occur.
• Licensure in multiple states strongly preferred; willingness to obtain additional licenses.

Responsibilities

• Conduct telehealth clinical evaluations to assess health needs and social drivers of health.
• Determine appropriateness for CHW services and refer for care coordination when indicated.
• Develop individualized care plans addressing clinical needs and social determinants of health.
