Alignment Healthcare, LLC is seeking a Remote Telehealth Coordinator to assist patients in accessing care through our Virtual Care Center. You will connect with patients by phone, collect symptom information, coordinate virtual appointments, and route concerns to the clinical team as needed.

Ideal candidates have healthcare experience, strong listening skills, and a passion for helping patients navigate telehealth.

Qualifications

• 1–2 years of experience in a primary care or medical office setting using phones and scheduling appointments.
• Experience providing high-quality customer service.
• Experience providing technical assistance.
• High school diploma or GED, required.
• Ability to work Saturdays.

Responsabilités

• Answer inbound calls in the Virtual Care Center queue.
• Gather symptoms, concerns, and patient information to support scheduling and routing.
• Escalate patient information to the appropriate nurse or clinical team per protocols.
• Schedule telephonic and virtual appointments with providers.
• Help patients prepare for virtual visits and navigate the telehealth platform.
• Support providers with referral and authorization requests, including submission and follow-up.
• Coordinate with IPA groups, PCP offices, pharmacies, and other health partners to support patient needs.
• Conduct outbound patient outreach (ER follow-ups, Teladoc follow-ups, annual assessments).
• Document patient interactions across multiple systems accurately.
• Assist with new hire onboarding through system training and shadowing.
• Complete other duties as assigned.
