Job summary

Beth Israel Lahey Health Primary Care in Needham, MA is seeking a Team Nurse RN to provide direct patient care in a busy outpatient setting. You will collaborate with physicians and MAs, coordinate care, and support preventive and chronic disease management under supervision of the Practice Manager and Senior Director of Nursing.

Responsibilities include triage by phone, administering vaccines, assisting with exams, documenting in the EHR, and training unlicensed staff.

Qualifications

• Graduate from an accredited Nursing Program required.
• Massachusetts RN license required and CPR certification.
• 1–3 years related work experience; new grads considered.
• CPR Certification.
• Strong interpersonal, patient communication and education skills.
• Proficiency with Microsoft Office applications (Outlook/Word/Excel/PowerPoint).

Responsibilities

• Coordinate and facilitate quality clinical management in outpatient care.
• Collaborate with providers to identify preventive care needs and manage chronic disease.
• Provide skilled telephonic triage and patient education.
• Perform direct patient care including AWVs and CCM.
• INR management and Coumadin teaching per protocol.
• Administer vaccines and medications per policy; educate patients.
• Document care in the EHR and schedule tests/referrals.
• Monitor abnormal results and follow up with providers.
• Oversee inventory and supervise MA staff; train staff.
• Maintain PHI security and report adverse events to Risk Management.
