Job summary

Beth Israel Lahey Health is seeking a Team Nurse to deliver primary care nursing in an outpatient setting. You will support physicians, provide direct patient care, supervise MAs and ensure coordinated care across the continuum.

The role includes education for patients and staff, triage by phone, vaccinations and wound care, with emphasis on safety, compliance and documentation in the EHR. This is a full-time on-site position in a busy primary care clinic.

Qualifications

• Graduate from an accredited Nursing Program; BSN preferred.
• RN or LPN license required; BLS certification required.
• 1–3 years related work experience.
• Strong interpersonal, patient education and communication skills.
• Proficiency with Microsoft Office (Outlook, Word, Excel, PowerPoint, Access).

Responsibilities

• Coordinate and facilitate quality clinical management across outpatient care.
• Collaborate with providers; assist exams, treatments and procedures.
• Provide telephonic triage, education, and care coordination.
• Oversee nursing activities, wound care and point-of-care testing.
• Document care in EHR and schedule referrals.
