Job summary

Qualifications

• Graduate from an accredited Nursing Program required.
• Bachelor's degree preferred.
• License Registered Nurse in Massachusetts required and Certificate 1 Basic Life Support required.

Responsibilities

• Coordinate and facilitate quality clinical management of a broad based outpatient population across the continuum of care.
• Collaborates with providers to identify needs for preventive care and chronic disease management.
• Provides patient education and self-management support.
• Performs nursing activities and documents care in the Electronic Health Record.
