Job summary

Qualifications

• Graduate from an accredited Nursing Program required.
• Massachusetts RN license and CPR/BLS certification required.
• 1–3 years related work experience preferred; new graduates considered.
• Strong interpersonal and patient education skills.
• Proficient with Microsoft Office and EHR systems.

Responsibilities

• Coordinate and facilitate quality clinical management of an outpatient population.
• Collaborate with providers to support preventive care and chronic disease management.
• Provide education and support for self-management goals.
• Perform nursing visits including AWVs and CCM as needed.
• INR management, vaccine administration and timely documentation in EHR.
