Job summary

Beth Israel Lahey Health seeks a Team Nurse RN to provide high-quality nursing care in our outpatient primary care setting. You will educate and support the MA staff, coordinate patient-centered care with providers, and contribute to safety and efficiency in a team-based model.

The role involves annual wellness and chronic care management visits, INR management, vaccines administration, and extensive patient education.

Qualifications

• Graduate from an accredited Nursing Program; Bachelor's degree preferred.
• Massachusetts RN license and BLS certification required.
• 1-3 years related work experience; new grads considered.
• CPR Certification required.
• Strong interpersonal and patient communication/education skills.
• Advanced MS Office / computer skills (Outlook, Word, Excel, PowerPoint, Access).

Responsibilities

• Coordinate and facilitate quality clinical management across outpatient care.
• Collaborate with providers; assist exams, treatments and procedures.
• Provide telephonic triage and care coordination.
• Perform nursing visits (AWVs, CCM) for BIDHC patients.
• INR management and teaching for Coumadin patients.
• Administer vaccines/medications per policy; educate patients.
• Document all patient care in EHR and schedule referrals.
