Job summary

Beth Israel Lahey Health is seeking a Team Nurse to provide RN/LPN level care in an outpatient primary care setting in Jamaica Plain. The role supports MAs, educates patients, and coordinates care under a collaborative team model with strong emphasis on safety and efficiency.

You will manage AWVs/CCM, assist with vaccines and medications, and ensure accurate EHR documentation while guiding unlicensed staff and patients toward better health outcomes.

Qualifications

• Graduate from an accredited nursing program.
• RN or LPN licensure is required, CPR/BLS certification.
• 1-3 years of related clinical experience, preferably primary care.
• Proficiency with Microsoft Office applications and web-based tools.

Responsibilities

• Coordinate and facilitate quality clinical management across outpatient care.
• Assist with exams, treatments, and procedures; provide triage by phone.
• Educate patients on self-management and preventive care.
• Monitor and manage CCM/AWV visits and INR/coumadin patients.
• Document care in the EHR; coordinate tests and referrals.
• Oversee supplies, PHI handling, and staff training needs.
