Job summary

Upward Bound House (UBH) seeks a compassionate Case Manager & Housing Navigator to support Transition Age Youth (TAY) in South Los Angeles in our Transitional Housing Program. The role focuses on building relationships and helping youth access permanent housing and resources.

You will provide life coaching, develop plans, coordinate housing options, and work with UBH and community partners to reduce barriers and promote independence.

Qualifications

• Bachelor’s degree in social work, human services or related field required; MSW preferred.
• Minimum two years of homeless services experience, preferably with Transition Age Youth (TAY).
• Experience with at-risk youth and housing, employment or education goals.

Responsibilities

• Provide life coaching and support services using strengths-based approaches.
• Develop service and housing plans with goals and action steps.
• Assist youth in identifying housing options and accessing services.
• Meet with youth weekly to monitor progress and barriers.
• Coordinate with UBH team and partners to create engaging activities.
• Maintain records and progress notes in HMIS and reporting systems.
