Job summary

Unity Care Group is seeking a Case Manager for Minors and Non-Minor Dependents in the Transitional Housing Program (THP) to provide direct support and intensive case management to Transitional Aged Youth (TAY) ages 16 to 21. The role collaborates with an interdisciplinary team to coordinate care and resources within the THP framework.

This position requires strong case management skills, knowledge of THP program statements, and experience with the TAY population.

Qualifications

• Bachelor's Degree in fields including counseling, social work or related area with licensure content for LPCC
• Alternative BA with 2 years in child welfare setting and experience with TAY 16–24
• CCL approval as an Excepted Case Manager prior to start date
• Experience working with the TAY population
• Knowledge of Microsoft Office applications
• Demonstrated case work, group work and care coordination skills

Responsibilities

• Provide direct support and intensive case management to Transitional Aged Youth (TAY) ages 16–21
• Coordinate care on an interdisciplinary team
• Utilize various approaches to establish relationships with youth and connect them with community resources
• Maintain weekly contact with youth to discuss goals and progress
• Ensure documentation, Medi-Cal paperwork and data entry are completed timely
• Develop care plans and monitor transitions to independent living
• Assist with transportation planning and access to services
• Attend team meetings and ensure compliance with THP Program Statement
