Job summary

Upward Bound House in Culver City is seeking a Housing Case Manager to support families experiencing homelessness by facilitating rapid access to permanent housing and supportive services.

You will provide comprehensive case management, link families to child care, employment, health, life skills, and aftercare, and develop Housing Stability Plans while coordinating with community resources. Strong communication and bilingual skills are required.

Qualifications

• Bachelor’s Degree in Human Services or related field is required.
• Minimum of two years experience serving homelessness or related population.
• Fluent bilingual English/Spanish preferred; strong communication skills.

Responsibilities

• Complete intake and assessment to determine housing needs.
• Identify and assist participants in securing housing and services; develop a Housing Stability Plan.
• Provide case management and referrals for childcare, employment, health and wellness, life skills, and aftercare.
• Meet participants weekly to monitor progress and address barriers.
• Maintain records via HMIS and generate regular progress notes.
• Develop relationships with landlords and agencies to access housing and supports.
