This role is essential in ensuring the effectiveness of the Supportive Services Division and its programs while working to improve the quality of life for older adults, adults with disabilities and their caregivers through access to comprehensive care. The person in this role will serve as the Supportive Services Specialist responsible for providing Care Coordination and Person-Centered Options Counseling. This role ensures compliance with the Department for Aging and Rehabilitative Services (DARS), agency policies, and program guidelines while providing high-quality care for older adults, adults with disabilities and their caregivers. This role will also collaborate with internal staff and external partners to address ongoing service needs and enhance program effectiveness.
High school diploma or G.E.D. and 5 years of experience in case management, service coordination, working with older adults, adults with disabilities and their caregivers who require community-based care services.
A successful candidate will have a bachelor’s degree in Behavioral Sciences, Human Services, Social Work, Gerontology, Education, Communications, Public Administration or a related field and 6 years experience in human services or programs related to older adults, and adults with disabilities. They will work effectively with internal staff, older adults, adults with disabilities, caregivers, families, and community partners. A successful candidate will demonstrate a strong ability to coordinate services, adhere to policies and procedures, and maintain compliance with program guidelines.
Monday through Friday, 8:30 a.m. to 5:00 p.m., with flexibility to adjust hours to ensure adequate staff coverage between the hours of 8:00 a.m. to 5:00 p.m.
$70,921.50 - $97,968.00 Annually
Full time positions with Prince William County Government qualify for Public Service Loan Forgiveness.
