Job summary

Prince William County seeks a dedicated Supportive Services Specialist to empower older adults, adults with disabilities, and their caregivers by connecting them with essential services. You will conduct in-home assessments, develop person-centered plans, coordinate with caregivers and partners, and provide ongoing support to help clients stay safely in their homes and communities.

Qualifications

  • High school diploma or GED with 5 years of relevant experience in case management or service coordination.
  • Bachelor’s degree in Behavioral Sciences, Human Services, Social Work, Gerontology, Education, Communications, Public Administration or related field with 6 years in human services.
  • Ability to coordinate services and comply with policies and program guidelines.
  • Experience with service coordination, client needs assessment, and linking to resources.
  • Bilingual ability preferred.

About This Role

This role is essential in ensuring the effectiveness of the Supportive Services Division and its programs while working to improve the quality of life for older adults, adults with disabilities and their caregivers through access to comprehensive care. The person in this role will serve as the Supportive Services Specialist responsible for providing Care Coordination and Person-Centered Options Counseling. This role ensures compliance with the Department for Aging and Rehabilitative Services (DARS), agency policies, and program guidelines while providing high-quality care for older adults, adults with disabilities and their caregivers. This role will also collaborate with internal staff and external partners to address ongoing service needs and enhance program effectiveness.

Typical Duties May Include (but Are Not Limited To)
  • Developing holistic person-centered care plans
  • Utilizing the Uniform Assessment Instrument (UAI) to determine service needs
  • Facilitation of service referrals
  • Collaborating with internal staff, and community partners to ensure the effective delivery of services and resolving concerns
  • Maintaining and managing accurate client records and data in electronic databases, ensuring compliance with privacy and security regulations
  • Critical thinking and problem-solving skills and ability to demonstrate exceptional customer service
  • Superior communication skills, both oral and written, to effectively engage with diverse populations
  • Strong organizational skills with attention to detail, flexibility, and a positive attitude
Minimum Requirements

High school diploma or G.E.D. and 5 years of experience in case management, service coordination, working with older adults, adults with disabilities and their caregivers who require community-based care services.

Preferences

A successful candidate will have a bachelor’s degree in Behavioral Sciences, Human Services, Social Work, Gerontology, Education, Communications, Public Administration or a related field and 6 years experience in human services or programs related to older adults, and adults with disabilities. They will work effectively with internal staff, older adults, adults with disabilities, caregivers, families, and community partners. A successful candidate will demonstrate a strong ability to coordinate services, adhere to policies and procedures, and maintain compliance with program guidelines.

Preferred Skills And Experience Include
  • 6 or more years of experience in program management, service coordination, working with older adults, adults with disabilities and their caregivers requiring community-based care services.
  • Experience in service coordination, including assessing client needs and linking them to appropriate resources
  • Proficiency in implementing policies, procedures, and process improvements to enhance program delivery
  • Developing holistic person-centered care plans
  • Experience with PeerPlace or a similar database system
  • Experience handling confidential information and documentation
  • Experience in Options Counseling
  • Bilingual ability preferred, but not required
Special Requirements
  • Must pass criminal background check pursuant to VA Code Ann. 19-392-92 (2010) and the Older Americans Act of 1965 as amended in Section 701
  • Must pass DMV Record check
  • Must possess a valid driver's license
  • Mandated reporter of elder abuse
  • Must take a health physical/TB test and be cleared for employment
  • Must Become Alliance of Information & Referral Systems (AIRS) Certified (must be attained within two years)
  • Certifications/Trainings required (or must be attained within twelve months) include:
  • Uniform Assessment Instrument (UAI) Training
  • CPR/First Aid/AED
Work Schedule

Monday through Friday, 8:30 a.m. to 5:00 p.m., with flexibility to adjust hours to ensure adequate staff coverage between the hours of 8:00 a.m. to 5:00 p.m.

Hiring Salary Range

$70,921.50 - $97,968.00 Annually
