• Required: Minimum one year’s professional experience with low-income, diverse populations in a social services setting.

Responsibilities

• Provide permanent supportive housing services to participants and assess eligibility.
• Perform intensive case management and develop individual housing service plans.
• Enter case notes and manage records in HMIS.
• Collaborate with housing partners and employment/education specialists to link clients to resources.

Education

Bachelor's degree in social work, public health, counseling, or related human services
