Job summary

LifeWorks is seeking a Case Manager to join the Housing team in Austin, TX. This is a full-time, non-exempt position offering community-based case management, client advocacy, and collaboration with service providers to support housing stability and well-being.

Qualifications

• Requires a Bachelor’s Degree in social or behavior sciences or administrative field.
• Minimum 2 years related experience; or equivalent combination.
• Lived experience of homelessness is a plus and may count towards qualifications.
• Proficient in HMIS, Microsoft Office, Outlook, and agency database.

Responsibilities

• Provide client-centered, healing-centered case management services to diverse individuals.
• Identify and leverage clients’ strengths for service planning and goals.
• Coordinate with teammates to ensure fidelity of the Case Management model.
• Assist with day-to-day needs and development of life skills (shopping, meals, budgeting).
• Assess mental/physical wellness and tailor wraparound supports and referrals.
• Manage incidents and crisis situations with de-escalation and safety planning.
