Qualifications

• Bachelor’s Degree in social or behavior sciences or administrative field required; minimum 2 years related experience or equivalent combination.
• Lived experience of homelessness is a plus and may count towards related qualifications.
• Proficiency with HMIS, Microsoft Office, Outlook, and agency databases is expected.

Responsibilities

• Provide client-centered, healing-centered case management to diverse individuals including those experiencing homelessness.
• Identify and leverage clients’ strengths to inform service planning and goals.
• Maintain timely, complete case records and ensure data accuracy in databases.
• Assist clients with daily needs and life skills such as budgeting, transportation, and housing search.
• Coordinate with partners to advocate for client needs and referrals, ensuring safety and crisis management as needed.
