Job summary

LifeWorks Austin, TX is seeking a Case Manager to join our Housing team. This role provides community-based case management, advocacy, and coordination of wraparound supports to help clients secure housing and stability.

Responsibilities include data entry, documentation, crisis intervention, and collaboration with partners. Requires a Bachelor's degree and at least 2 years of related experience; living experience with homelessness is a plus.

Qualifications

• Bachelor's degree in social or behavior sciences or administrative field
• Minimum 2 years related experience or equivalent
• Lived experience of homelessness is a plus and may count towards related qualifications
• Proficient in HMIS, Microsoft Office, Outlook, and agency database

Responsibilities

• Provide client-centered case management to diverse individuals who may have experienced homelessness
• Identify and leverage clients' strengths to inform service planning
• Work with teammates to ensure fidelity to Strengths Model of Case Management
• Assist with day-to-day needs and development of life skills
• Assess clients' wellness and coordinate tailored wraparound supports
• Manage incidents and crises with de-escalation and safety planning
• Engage with community partners to advocate for client needs and referrals
• Help clients build a network of support and relationships
• Maintain accurate case records and timely data entry
• Assist clients in obtaining documents for employment or housing
• Coordinate benefits information and referrals as needed
• Ensure informed consent and program understanding
