Job summary

Caritas of Austin is seeking a Referral & Case Management Specialist in Austin, TX. This full-time, grant-funded role coordinates client intakes, assesses eligibility, and provides ongoing case management to promote housing stability and self-sufficiency.

The specialist serves as the first point of contact, links clients to internal and external resources, and maintains timely documentation in HMIS. The position requires a Bachelor’s degree (Master’s preferred) and at least one year of

Qualifications

• Bachelor’s degree from an accredited college or university; social work/public health/counseling or related fields preferred.
• Master’s degree in social work, psychology, public health or related field preferred.
• Minimum one year of experience with low-income, diverse populations in a social service setting.

Responsibilities

• Coordinate client intakes and assess eligibility for services.
• Maintain HMIS case notes and documentation in a timely manner.
• Develop individualized housing service plans and assist with housing identification.
• Collaborate with community partners to connect clients to housing and employment resources.
