Job summary

Care Lync is seeking a dedicated Support Coordinator to help adults with intellectual, developmental, and physical disabilities live independently. You will assess needs, design individualized plans, coordinate with providers, and advocate for clients to secure the services they require.

The role emphasizes collaboration, ongoing training, and career growth within a supportive team dedicated to dignity and community integration.

Qualifications

Strong organizational, communication, and advocacy abilities.
Comfortable traveling up to 50% to meet participants in homes and communities.
Valid driver’s license, auto insurance, and reliable transportation.
Commitment to required certifications and state clearances.

Responsibilities

Conduct person-centered assessments to identify needs, preferences, and goals.
Develop and regularly update Individual Support Plans (ISP).
Provide participants with choice and advocacy in care options.
Coordinate with medical and behavioral professionals, families, and community resources.
Monitor services to ensure safety, satisfaction, and progress toward independence.
Assist aging individuals and those with disabilities in accessing community resources.
Maintain documentation, confidentiality, and program compliance.
Engage in ongoing training and professional development.
