Job summary

Care Lync, a social services agency, is looking for a Support Coordinator to develop and implement individualized care plans, coordinate services, and monitor outcomes for adults with IDD, autism, and related disabilities. The role involves brokering services, ensuring access to housing, medical, social, and community supports, and maintaining client confidentiality.

Qualifications

BS/BA degree in related course work.
Experience with Microsoft Office and state-maintained databases.
Maintain CPR/First Aid certification and state-related Clearances.
Valid Driver's License with personal vehicle insurance coverage.
Complete and maintain required training courses and certifications.
Inform supervisor immediately when any change in auto insurance policy, vehicle registration or driver’s license status.

Responsibilities

Complete assessments to identify participant needs and desires for service delivery.
Develop a Person-Centered Service Plan for each participant and update at least annually.
Ensure participant choice of providers by giving information for informed decisions.
Ensure the Behavioral Support Plan and Crisis Plan align with the ISP.
Coordinate with care team members to ensure seamless service delivery.
Travel to participants as needed (up to 50%).
