Care Lync is seeking a dedicated Support Coordinator to guide adults with disabilities and aging adults through available services, housing, medical care, and community resources. You will advocate for participants, develop and update care plans, and coordinate with professionals to ensure high-quality, person-centered support.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• 3+ years of experience providing case management or support coordination to individuals with disabilities and/or aging adults.
• Strong organizational, communication, and advocacy skills.

Responsibilities

• Conduct person-centered assessments to identify needs, preferences, and life goals.
• Develop, implement, and update Individual Support Plans (ISP).
• Provide participants with choice and advocacy in providers and care options.
• Coordinate with medical professionals, behavioral specialists, family members, and community resources.
• Monitor and evaluate services for safety, satisfaction, and progress toward independence.
• Maintain documentation, confidentiality, and compliance with program standards.
