Job summary

Care Lync is seeking a Support Coordinator to empower adults with intellectual and developmental disabilities and aging individuals to access housing, medical care, community resources, and independent living supports in their communities.

You'll conduct person-centered assessments, develop and update Individual Support Plans, coordinate with medical and behavioral professionals, and advocate for participants while maintaining documentation and confidentiality in compliance with program

Qualifications

Bachelor's degree in social work, psychology, human services, or related field.
No experience required.
Ability to travel up to 50%.

Responsibilities

Conduct person-centered assessments to identify needs, preferences, and goals.
Develop, implement, and update Individual Support Plans to ensure appropriate services.
Provide participants with choice and advocacy in provider and care options.
Coordinate with medical professionals, behavioral specialists, family, and community resources.
Monitor services to ensure safety, satisfaction, and progress toward independence.
Maintain documentation, confidentiality, and compliance with standards.
