Job summary

Care Lync is seeking a dedicated Support Coordinator to empower adults with disabilities and aging adults. You will assess needs, develop ISP plans, coordinate with providers, and advocate for participant choice, while traveling up to 50% to meet clients in homes and communities.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational and advocacy skills.
• Willingness to travel up to 50% to meet clients in homes and communities.

Responsibilities

• Conduct person-centered assessments to identify needs, preferences, and life goals.
• Develop, implement, and regularly update ISP to ensure participants receive the right services.
• Provide participants with choice and advocacy, helping them select providers and care options.
• Coordinate with medical professionals, behavioral specialists, family members, and community resources.
• Monitor and evaluate services to ensure safety, satisfaction, and progress toward independence.
• Support individuals with disabilities and aging adults in accessing community resources.
• Maintain documentation, confidentiality, and compliance with standards.
• Participate in ongoing training and professional development to strengthen skills.
