Qualifications

• Commitment to maintaining required certifications and state clearances.

Responsibilities

• Conduct person-centered assessments to identify needs and goals.
• Develop, implement, and update Individual Support Plans (ISP).
• Provide participants with choice and advocacy for care options.
• Coordinate with medical professionals, specialists, family, and resources.
• Monitor services for safety, satisfaction, and progress toward independence.
