Job summary

Care Lync in the United States is seeking a Support Coordinator / Case Manager to empower adults with disabilities and aging individuals to access housing, medical care, community resources, and independence. You will assess needs, develop and monitor ISPs, and advocate for participants’ choices in a compassionate, person-centered approach.

Travel up to 50% to meet participants in homes and communities, maintain documentation, and ensure compliance with program standards while pursuing ongoing

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Comfort working independently and traveling (up to 50%) to meet with participants in their homes and communities.
• Valid driver's license, auto insurance, and reliable transportation.
• Commitment to maintaining required certifications and state clearances.

Responsibilities

• Conduct person-centered assessments to identify needs, preferences, and goals.
• Develop, implement, and update Individual Support Plans (ISP).
• Provide participants with choice and advocacy in providers and care options.
• Coordinate with medical professionals, behavioral specialists, family, and community resources.
• Monitor and evaluate services for safety, satisfaction, and progress toward independence.
• Support individuals with disabilities and aging population in accessing community resources.
• Maintain documentation, confidentiality, and program compliance.
• Participate in ongoing training and professional development.
